Free NHS Prescriptions: Who Qualifies and How to Check Your Eligibility

In England, the standard prescription charge is £9.90 per item (2025/26 rate). However, a large portion of the population is automatically exempt and pays nothing. You are entitled to free prescriptions if you are: under 16 years old; aged 16 to 18 and in full-time education; aged 60 or over; pregnant or have given birth in the last 12 months (you need a Maternity Exemption Certificate — apply via your midwife or GP); have a qualifying medical condition such as diabetes requiring medication, hypothyroidism, epilepsy, or a permanent fistula; have a continuing physical disability that prevents you going out without help; or hold a valid HC2 certificate (full help with health costs for low-income individuals). In Wales and Scotland, all prescriptions are free for everyone. If you live in those nations, you pay nothing regardless of age or income.

If you are on a qualifying benefit, you receive free prescriptions automatically. These benefits include Universal Credit (in most cases), Income Support, income-based Jobseeker's Allowance, income-related Employment and Support Allowance, and Pension Credit. If you are not on these benefits but have a low income, you may still qualify for help through the NHS Low Income Scheme. Apply using form HC1 (available from GP surgeries, pharmacies, Jobcentres, and online at NHSBSA). If successful, you receive an HC2 certificate which entitles you to free prescriptions, dental treatment, eye tests, and more. The threshold for eligibility is based on your income and savings — generally, if your savings are below £16,000 and your income is low, you are likely to qualify. Apply even if you are unsure — there is no penalty for applying and not qualifying.

If you do not qualify for free prescriptions but take regular medication, a Prescription Prepayment Certificate (PPC) could save you a significant amount each year. A PPC covers unlimited prescription items for a flat fee: currently around £31.25 for three months or £111.60 for twelve months (2025/26 rates). If you regularly need two or more prescription items per month, the annual PPC pays for itself very quickly. You can buy a PPC online through the NHS Business Services Authority (NHSBSA) website or by calling 0300 330 1341. You can also spread the annual cost by setting up a Direct Debit, paying in ten monthly instalments. Keep your PPC in your wallet and present it every time you collect a prescription. This is one of the most straightforward money-saving steps for anyone regularly visiting a pharmacy — yet many people who would benefit enormously simply do not know it exists.
